SUMMER CAMP 2024

I/We the undersigned hereby certify that I/We am/are the parent(s) or legal guardian(s) of the clinic/tryout participant. 

Consent for Medical Treatment:
I/We hereby give permission for the staff of LEGENDS FC  to seek appropriate medical attention for the player and for the medical attention to be given and for the player to receive medical attention in the event of accident, injury or illness. I/We will be responsible for any and all costs of medical attention and treatment, except for that covered by the camp's excess medical coverage policy.

Release For Communicable Diseases Including COVID-19:
I am aware there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from an outbreak of any and all communicable disease, including but not limited to, the virus “severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)”, which is responsible for Coronavirus Disease (COVID-19) and/or any mutation or variation thereof. 

I willingly agree to comply with the stated and customary terms and conditions for participation stablished by Tigres FC as regards protection against infectious diseases. If, however, I observe any unusual or significant potential hazard during my presence or participation in the program, the player will remove himself/herself from participation and bring such to the attention of the nearest member of the LEGENDS FC staff immediately.

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS THE Tigres FC program, its directors, coaches, officials, employees, other players/families, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event from any and all claims, demands, losses, and liability arising out or related to any ILLNESS, INJURY, DISABILITY, OR DEATH I may experience, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES or otherwise, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
FOR PARENTS/GUARDIANS OF PARTICIPANT UNDER AGE 18 (MINOR) AT TIME OF REGISTRATION 

This is to certify that I, as parent/guardian with legal responsibility for this Participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify and hold harmless the Releasees from any and all liability incidents to my minor child’s involvement or participation in these programs as provided above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law.


Athletics Release: 
I/We hereby acknowledge that our child is physically fit and mentally capable of participating in soccer training activities. I/we hereby acknowledge that playing or practicing soccer is a potentially hazardous activity. I/we agree that I/we am/are voluntarily entering and assuming all risks associated with participating in those activities, including but not limited to falls, contacts with other participants, the effects of weather including heat and or humidity, and the conditions of the training grounds and facilities, all risks being understood and appreciated by you. I/We, the undersigned for ourselves, our heirs, executors and administrators hereby waive, release, and forever discharge LEGENDS FC and PREMIER SOCCER FIELDS LLC staff, officers, agents, employees, representatives and successors and assign of and from all rights and claims for damages, injury or loss to person or property which may be sustained or occur during participation in training activities or while at training, whether or not damages, injury or loss is due to negligence. 

Use Material for Promotional Purposes
I/We hereby give permission to LEGENDS FC and PREMIER SOCCER FIELDS LLC to use pictures and videos of the players to promote future events organized by the company.  

Weather Policy:
In the event of inclement weather once training begins, all players will be asked to leave the field to the designated shelter area.  If training is cancelled once started, you will be asked to return to the field and pick up your player.

Cancellation & Refund Policy
No Refunds are given for any reason. In the event of extreme weather or other unforeseen circumstances causing the camp to be cancelled, you will be informed via email and/or text message.

With the payment you are accepting accept these policies.
